Integrative health learning collaborative faculty member.
Elena was a faculty member of the 2020 Samueli and FMEC(Family Medicine Educational Consortium) Integrative Health Learning Collaborative led by Dr. Wayne Jonas. She supported practices in implementing the HOPE toolkit as a way to operationalize whole person care. The HOPE Toolkit, created by Dr. Jonas, was adapted from VA’s Whole Health Personalized Health Inventory to be used outside of VA systems. As part of the learning collaborative, residencies, health systems, FQHCs and other practices implemented the tools. Elena facilitated some large group sessions, conducted small group breakout discussions and one-on-one consultations with participant practices. Elena also created learning resources and presentations.
Published articles on the project are found in the Journal of the American Board of Family Medicine, “Implementing Whole Person Primary Care: Results from a Year-Long Collaborative” and Annals of Family Medicine, “Implementing Whole Person Primary Care.”
Elena currently leads the 2024-25 FMEC Whole Person Care Learning Collaborative which provides practice improvement and CME credit to participating physicians implementing the HOPE Toolkit.
